My name is Anthony J. Worley Sr. I was a resident of Frankford Nursing Home at the age of 38 because a drug addition and alcohol habit caused me to develop a blood clot. Complications from the clot caused me to develop a contracture of the knees. I was in the Nursing Home for two and a half years, long after becoming clean & sober. I did not think I could get out of the Nursing home because of my disability, I did not know that I could live on my own with assistance. I was hurting so bad in my heart, my family turned their back on me. The Nursing Home was my home until a group of people came into my life from the Maryland Disability Law Center, (MDLC) and The SunShine Folk. (SSF) Ms. Kim Stevens from MDLC and Mr. Floyd Hartley from the SSF came to visit me in the Nursing Home. They informed me of the Medicaid Community Choices Waiver program that allows a nursing facility resident whose services are paid for by Medicaid to receive their services in the community. They put me in touch with Mrs. Hilary Christian of the Coordinating Center.  Ms. Christian told me to make a plan for what I would need to move from the Nursing Facility to the community. Then we started working on getting the things on my list. I also applied for a housing voucher, I was approved for the voucher. These three groups worked together to help me move from the nursing facility to the community with the supports I needed to live independently.  Now I am living in the community in my own apartment.  I have my life back.

Don’t give up, you can have your own place to live too, you just have to believe. There is help for you.  I LOVE YOU ALL AND THANK YOU SO MUCH !!
